
Vision
Over 220 000 Canadians die each year, of these it is estimated that only 5% receive integrated and interdisciplinary palliative care .
Our vision is to enable those with a life-threatening illness, if they wish, to be cared for at home receiving the best possible palliative care. We offer the support needed by family caregivers and health professionals to make it easier for them to cope during a difficult time.

Providing reliable, current, and practical information to reduce the fear, isolation, and powerlessness that can overwhelm those who are thrust into providing palliative and end-of-life care in the home. |

Identifying both consistent and ever changing needs of end-of-life caregivers and their families. |

Determining the gaps between those needs and current community services and programs. |

Bridging gaps in the existing system by advocating on behalf of those who seek support; and helping them find quick access to the appropriate resources. |

Developing and offering, without charge, practical, reliable, and up-to-date programs and services for caregivers, their families and friends, students, and the community at large. |

Complementing and working with existing service providers, community organizations, and post-secondary institutions. |

Serving as a model and leader in the field of palliative and end-of-life care, to guide other communities in establishing similar centres and associated resources. |
